This form must be completed by a member of Barts staff or a professional from another organisation working to support a patient of St Bartholomew’s Hospital. We cannot accept applications directly from patients.

The Fund has a limited budget for patient grants which cover a wide variety of needs and must apply funds sparingly. We are here to help patients as a last resort when there is no alternative way of meeting their need.
There must be both a clinical or therapeutic need for the item or funds requested AND a clear financial reason why the patient cannot afford to meet these costs themselves. Decisions are made based on the information provided in the application form.
To ensure that our trustees can effectively consider grant applications for patients, please complete the application form with as much detail as possible. If it is felt that more information is needed, we will ask further questions about the beneficiary via the person making the referral.
